YOUR PERSONALIZED CARE INCLUDES:

• Teamwork together with you, your RN Care Manager, Physical Therapist, Occupational Therapist, Medical Social Work, Speech Therapist, Community Liaison Expert, Dietician, and Bath Aide will work together to coordinate a Plan of Care to achieve your goals.

• Assessment/Reassessment of functional activities, body mechanics, health history, and more.

• Education/Information including proper body mechanics, home exercise plan, progress, and more.

• Plan of Care including treatment, goals, overall wellness, and listening to your needs.

• Follow-Up

• Equipment incorporated into your Plan of Care may include a walker, grab bars, shower chair, hospital bed, commode, oxygen equipment, wheelchair, gait belt, and more.
